Osteosarcoma

Definition: Osteosarcoma is a type of bone cancer that develops in the cells that form the bones. It is the most common primary malignant bone tumor and generally affects children and young adults, especially during periods of rapid bone growth.

Causes

Genetic Factors: Certain hereditary conditions, such as Li-Fraumeni syndrome and retinoblastoma, increase the risk of developing osteosarcoma.

Predisposing Factors: Inherited genetic faults or previous radiation exposure may make an individual more susceptible to osteosarcoma.

Symptoms

Pain: Persistent bone pain, often worsened by movement or at night, is a common symptom of osteosarcoma.

Swelling and Lumps: Swelling or the presence of a palpable mass near the affected bone may indicate the presence of osteosarcoma.

Fractures: Osteosarcoma weakens the affected bone, making it more susceptible to fractures or breaks, even with minor trauma.

Limited Mobility: As the tumor grows, it can restrict the range of motion and cause difficulty in moving the affected limb.

Diagnosis

Physical Examination: A thorough examination of the affected area along with a detailed medical history is the initial step in diagnosing osteosarcoma.

Imaging Tests: X-rays, CT scans, MRI, and bone scans are commonly used to visualize the tumor and determine its size, location, and extent of spread.

Biopsy: A biopsy is crucial for confirming the presence of osteosarcoma. It involves removing a small portion of the tumor or the entire tumor for laboratory analysis.

Treatment

Surgical Intervention: The main treatment for osteosarcoma involves surgical removal of the tumor, along with the surrounding healthy tissue to minimize the risk of recurrence.

Chemotherapy: Chemotherapy drugs are often administered before and after surgery to destroy any remaining cancer cells and reduce the risk of metastasis.

Radiation Therapy: In some cases, radiation therapy may be used in conjunction with surgery and chemotherapy to target and kill cancer cells that may have spread to nearby tissues.

Limb Salvage Surgery: When possible, surgeons may opt for limb-sparing procedures to remove the tumor while preserving the affected limb’s function and appearance.

Prognosis

Overall Survival: The prognosis for osteosarcoma has improved significantly over the years, with current 5-year survival rates between 60% – 70% for localized tumors.

Metastasis: The presence of metastases at the time of diagnosis reduces the 5-year survival rate to approximately 20 – 30% depending on the extent of spread.

Long-Term Effects: Survivors of osteosarcoma may experience long-term side effects such as reduced bone strength, joint problems, or secondary cancers due to previous treatments.
